Before sending Client Intake forms to Bankruptcy Coordinator for processing, please do the following:

(1) Download a free set of Client Intake forms.
(2) Have your clients fill them out.
(3) Please review completed forms to make sure all areas are properly completed.
(4) Fax the Client Intake forms and supporting documentation to our e-fax at 727-231-9514.
(5) All questions answered with a "yes" on the Statement of Affairs should have a detailed answer accompanying the question.
(6) The Income form should be compared to an actual debtor paycheck stub to verify the amounts are accurate. Please include copies of the six most recent paycheck stubs with the Client Intake form.
(7) Obtain a copy of the debtor's driver's license and verify the social security number they provided on the general information form.
(8) If there are any pending lawsuits, foreclosures or wage garnishments, a copy of the court pleading should be included with the complete set of Client Intake forms.

NOTE: Bankruptcy Coordinator initially prepares your client's petition within hours of receiving documentation. The only delays we experience occur when the client does not provide the critical information necessary to prepare their petition or they do not thoroughly complete the Questionnaire.
